What is a DIEP flap breast augmentation?
The DIEP flap breast augmentation is a surgical technique that uses your own body tissue to naturally enlarge the breasts without the use of implants. DIEP stands for Deep Inferior Epigastric Perforator. In this procedure, skin and fat from the lower abdomen are moved to the breasts to create additional volume, without using implants. While the DIEP flap method has been used for many years in breast reconstructions following breast cancer, this technique can also be applied for cosmetic breast augmentation. Instead of placing implants, the surgeon uses the fat tissue from the lower abdomen to enlarge the breasts.

The FEMALE augmentation is the most natural breast enlargement available today with guaranteed results. It is a complex procedure consisting of multiple stages, but it can be explained quite simply. In the FEMALE augmentation, the specialized plastic surgeon removes a portion of your abdominal fat roll and attaches this skin flap, including the fat tissue and blood vessels, to the desired area in the breast. The skin flap from the transplanted tissue is removed three days after the surgery. In addition to breast augmentation, a tummy tuck is also included in this procedure.

The consultation from A to Z
You will come to MOOI Clinic in Maastricht to discuss your breast augmentation without implants. During the consultation, you will speak with the plastic surgeon who specializes in this particular treatment. From general information about pre- and post-care to details such as the flap, scars, and expected results.
During the consultation, we review what is possible and take necessary measurements for possible surgery. The plastic surgeon may also inform you about alternative treatments. It is very important to us that you feel good about your choice. Today, we will also take photos for your file.
After the consultation, you will be aware of everything about your future natural breast augmentation: of our honest advice, expected results, risks, recovery and costs.
There is a consultation fee of €200 for this appointment, which will be deducted from the treatment price.
The operation
Before the surgery can be performed, additional tests are required. This includes examining your blood vessels to assess whether you are eligible for this surgery. The FEMALE augmentation will be done under general anesthesia, so you won’t feel anything during the procedure.
The anesthesiologist will put you under anesthesia in the OR. The surgery is only performed in the hospital. Exactly how and what will happen during surgery depends, of course, on your body, wishes and choices.
The FEMALE augmentation is a combination of treatments:
- A breast augmentation including contour correction.
- A breast correction (lift and augmentation) including contour correction.
Depending on the procedure performed, the surgery takes about 6 to 8 hours. A minimum of two overnight stays in the hospital are required.
Aftercare and recovery
After the surgery, you will stay in the hospital for at least two nights. On the third day after the surgery, the skin island (taken from your abdomen) is removed from the breast. This is done under local anesthesia. Any drains will also be removed. You also need to remain on bed rest at home for another 2 days. During bed rest, you will receive injections to prevent thrombosis.
You may have some after pain. You will need an abdominal belt and a firm sports bra, which you must wear day and night for up to six weeks after the surgery. This stops swelling and fluid accumulation. If necessary, drains provide drainage of wound fluid. These are removed after a few days. You may begin walking carefully short distances in a slightly hunched posture starting on the third day.
Your recovery takes six weeks. We will discuss the guidelines and recovery process during the consultation.
The benefits:
In the Netherlands, there are two types of breast augmentations: with implants and lipofilling (using your own fat). The FEMALE augmentation is an existing technique from reconstructive surgery that is now offered by MOOI Clinic with a different purpose, namely to enlarge your breasts rather than reconstruct them. This breast augmentation has several benefits:
- 100% your own body material.
- Your breast feels natural and looks natural as well. Additionally, it is possible to transplant a nerve from the abdomen to the breast to ensure that the sensation remains as intact as possible.
- The result is immediately visible, with no volume loss as seen in lipofilling, where 30-50% of the fat can be lost.
- You can increase by several cup sizes, whereas lipofilling typically results in an increase of only one cup size. This depends on the amount of fat available in the lower abdomen.
- Lifetime enjoyment from just one treatment! No need for implant replacement or removal.
- The tissue is placed in the breast as a prosthesis, en bloc, making it easier to identify during mammograms.
The drawbacks:
As with any procedure, there are also drawbacks to a FEMALE augmentation. We believe it is very important for women to make an informed decision.
- Duration: The surgery takes longer than an average breast procedure. You should expect a surgery lasting 6 to 8 hours.
- Scar on the abdomen: The abdomen becomes flatter, but a scar will form from flank to flank (similar to a tummy tuck). Depending on the location of the blood vessels, the scar may be slightly higher than the bikini line.
- Size: The size of the breast depends on the amount of tissue available on the abdomen.
- Sometimes multiple procedures are required. Small corrections may be needed, as the transplanted tissue cannot be overly manipulated right away.
Good to know
As with any procedure, there is a risk of bleeding, infection, or impaired wound healing with a FEMALE augmentation. This is usually easily remedied. Given the size of the scars, there is a chance that these problems will occur. It is also important to be aware that this procedure takes longer than a standard breast correction, typically around 6-8 hours. You may experience (temporary) numbness around the scars.
This is normal and part of the procedure. This area is getting smaller but will not disappear completely. Most people do not find it disturbing. Any small folds in the skin disappear spontaneously over time.
Scar recovery can never be completely predicted. The appearance of thick and lumpy scars, also called scar hypertrophy, can occur. Sometimes a scar correction can then give improvement after a year.
Fat necrosis may occur; the death of fat tissue under the skin (1-3%). This is especially common in clients with high body weight. In some cases, this may lead to infection and wound dehiscence, and in the worst case, the transplanted tissue may need to be partially or completely removed. Drainage and administration of antibiotics may also be necessary.
The abdominal muscles may become slightly weaker, which can lead to a bulging effect (a slightly rounder lower abdomen).
QUIT SMOKING
We advise all our clients to quit smoking around the time of treatment. The same applies to the use of nicotine patches and gum. Smoking not only makes you less MOOI on the outside, but also on the inside. If you smoke, you have a higher risk of complications such as poor wound healing, skin shedding or respiratory problems with general anesthesia. In addition, smoking has a detrimental effect on the ingrowth of fat.

Stefania Tuinder, Associate Professor at MUMC+ in Maastricht, has been working in the Netherlands since 2005. She completed part of her training here and part of it in Italy. She has conducted extensive anatomical research on new techniques for breast reconstruction using autologous tissue. She developed techniques such as the sc-GAP/sc-GAP make-over and LTP flaps. Her main research focus at the moment concerns the sensitivity of the breast. She has over 70 peer-reviewed publications to her name and is the author of several chapters on breast reconstruction in 5 books.
